			<content:encoded><![CDATA[<p>Medical billing outsourcing, also known as third-party medical billing, is regulated by the Office of the Inspector General (OIG). The OIG was created by Congress through the Inspector General Act of 1978 in response to a series of government scandals that occurred within nearly every major Federal Agency throughout the 1960s. Lawmakers believed that by creating an independent Office of Inspector General for each Federal Agency; the government would be better able to detect fraud and prevent the waste and abuse of Federal taxpayer monies. The hope was that these Federal watchdogs would reestablish the public’s confidence in government agencies.</p>
<p>The Office of the Inspector General, in conjunction with the U.S. Department of Health and Human Services, developed and released the Compliance Program Guidance for Third Party Medical Billing Companies on November 30, 1998. The intent of this program is to promote a higher level of ethical and lawful conduct throughout the entire health care industry; and to protect the solvency and stability of the Medicare Trust Fund (also known as Medicare).</p>
<p>Before we go on any further don’t get the impression that the U.S. Department of Health and Human Services (HHS) carries some sort of grudge against or questions the validity of medical billing outsourcing services. Prior to the Compliance Program Guidance for Third Party Medical Billing Companies was established, the HHS had already issued compliance programs for the clinical laboratory and hospital industries as well as home health care agencies.   </p>
<p>The legislation comes in response to the proliferation of medical billing out sourcing companies throughout the health care industry; the undo influence these medical billing services could have on a physician’s billing and coding practices; and the potential negative effects of unscrupulous billing and coding practices by these medical billing services on Medicare. </p>
<p>Like most OIG-issued compliance programs, conformity to the Compliance Program Guidance for Third Party Medical Billing Companies is strictly voluntary. On other hand, issues that arise due to a failure on the part of the medical billing service to abide by these “voluntary” compliance programs could expose the billing service to legal actions brought on by the U.S Government. Protect yourself and your business by interpreting “voluntary” as “mandatory” or be prepared to suffer the consequences. </p>
<p>The OIG has identified seven fundamental elements to an effective compliance program. They are:</p>
<p>1.	Implementing written policies, procedures and standards of conduct;</p>
<p>2.	Designating a compliance officer and compliance committee;</p>
<p>3.	Conducting effective training and education;</p>
<p>4.	Developing effective lines of communication;</p>
<p>5.	Enforcing standards through well publicized disciplinary guidelines;</p>
<p>6.	Conducting internal monitoring and auditing; and</p>
<p>7.	Responding promptly to detected offenses and developing corrective action.</p>
<p>In future articles I will continue to explain the Compliance Program Guidance for Third Party Medical Billing Companies in plain English so you can take the steps necessary to protect your medical billing service from unwanted government intervention. </p>

			<content:encoded><![CDATA[<p>Mobile home park tenants are not rich. Most of them live from paycheck to paycheck. As a result, they frequently don’t pay their bills on time – sometimes at all. To motivate these tenants to pay their lot rent on time, you must enact a late fee for rent that is not received by the due date. However, enacting such a plan is a lot more complicated than most park owners recognize. And messing up the plan can cause extreme legal and financial penalties. Here are a few initial points to consider: <br/><br/>How much to charge the tenant. <br/><br/>There is a law in most states as to the maximum late fee you can charge. It is not left up to your discretion. You are not allowed to charge a punitive amount. For example, if the lot rent is $150 per month, your late fee cannot be $100. The law is very specific on what you can and can’t charge. Don’t know the maximum amount allowed by law? You’ve got to get this data before you can go forward. <br/><br/>How much to charge the tenant as long as it is within the law. <br/><br/>You do not want anyone to ever be late. As a result, you should charge the maximum amount allowed by law to definitely get their attention. If the maximum is $50, then charge $50. I’ve toyed with this as much as anyone, but I’ve found that you have to make it absolutely not an option to be late, or the tenant may rearrange his payment plan and pay for that needed car repair/case of beer/cell phone bill before your lot rent. I cannot think of any reason not to go for the full amount allowed by law. <br/><br/>When do your charge it? <br/><br/>You should charge the late fee after a certain grace period. For example, if the rent is due on the first of the month, then you might have a grace period of the 5th. Any rent paid between the due date and the grace period (and obviously before the due date) would not be assessed any type of late fee. However, any rent received after the grace period would receive a late fee. In our example, any rent received on the 6th or later would be charged a late fee. <br/><br/>How do you prove when you got it? <br/><br/>The best way to do this is by postmark, assuming that you have the rent sent in to a P.O. Box as we do. If the postmark is after the fifth, then you will charge a late fee. What if the postsmark is on the fifth? Well, in some areas, if you sent it on the 5th, it can still reach its destination theoretically by that afternoon. So you are much safer just using the day after the end of your grace period for the postmark definition of late rent. And obviously, you want to save every late postmarked envelope as Exhibit A if you have to go to court over it. No judge is going to rule against you if the postmark is later than the grace period end date. <br/><br/>What about a late fee system that increases with every day?  <br/><br/>These systems, and we’ve tried them, are just too complicated. Although you may feel like it is going to motivate the customer, we’ve found that it really doesn’t – they don’t think that strategically. Basically, if they have the money in hand they’ll pay you, and if not they can’t. It’s not like you are reminding them. Normally, if they miss the first of the month, they don’t get paid again until the 15th, and as a result can’t pay you again until the fifteenth, no matter what the penalty. Just keeping track of a daily escalating late fee will cost you way more in time than it is worth. <br/><br/>How do they know they owe a late fee for next month?  <br/><br/>The best system is to send a monthly invoice, showing the rent plus a late fee, if they have one. Obviously, you have to have some kind of notification system if you want to be paid. If you let the tenant pay the rent in person at the park office, then the manager will need to keep a list of who owes it and collect at that time. If you send the rent to a P.O. Box, then there will have to be some type of system in place or you will never get your late fees. They can’t pay it if they don’t know they owe it. And don’t imagine that they should know themselves – it doesn’t happen in the real world. They always dream that somehow they got around the system, or you screwed up and forget to assess it. <br/><br/>Am I being mean charging a late fee? <br/><br/>No. On the contrary, you are being a bad landlord if you don’t. If the general tenant base starts delaying or stops paying altogether their rent, then the property will either go bankrupt or into disrepair. Neither scenario is for the good of the community. This is very important, as it theoretically eliminates your line item of bad debt, when offset by late fees. Without late fees, you will never have perfect collections. With late fees, you scientifically can. And that’s essential for hitting your budget. <br/><br/>Conclusion <br/><br/>Late fees are an essential part of being a good landlord. And it is very important that you do them the right way for them to be fair and accurate. 			<content:encoded><![CDATA[<p>There is nothing worse than being unable to move about freely for months on end, especially for somebody addicted to sports. When my friend was unfortunately injured during his motor car racing accident, fracturing both his legs and his backbone, he could only stay at home with the least possible physical movement. He was almost a person paralyzed with Since it was mandatory for us to leave for work every morning, he was left alone at home for the entire day. The only person who he managed to have for company was this depressing nurse. She would come, serve him and then leave. The days were a drag and he complained that he was ‘beginning to forget what the rest of the house looked like’! The situation was sad, and we all felt sorry for him. But how could he be helped? The only way to recover after the operation was to restrict body movement, especially the movement of his legs, since they were badly affected in the accident.However, it was only after two weeks of staying bed ridden when he had had enough. He could not take it anymore. Although he didn’t have any other option except for staying on the bed for another six months as prescribed by the doctor, the mental trauma was already reaching its peak!  The worst case scenario was whenever he needed the nurse to help him in the bathroom and the toilet. He was losing his head, and it was only the very beginning of the six months of hardship.Finally we decided that there ought to be a way out. If he had to go on for as long as six months this way, he would definitely start suffering from depression. And who wouldn’t? Six months is a long time to have to stay in bed in the same old position, and having to depend on somebody else for something as basic as the bathroom and toilet. So we started searching the internet to find out if there was any way to save his sanity!Finally one of us came across this thing called a manual wheelchair. It promised to help him move about easily around the house, however, the problem was that it needed human muscle power to move around, and our friend was quite weak following the surgery. The nurse suggested that we could opt for an electric wheelchair, but the prices were high. Eventually we managed to get a  that could be wheeled around with the use of electricity, at an affordable rental price and representing great value for money. Gradually we learnt about the wheelchair  that could be used to help solve my friend’s bathroom and toilet troubles to a great extent.Though I would not say that the wheelchair was easy to use during the early phases of recovery, my friend was definitely feeling a lot more liberated and mobile than he had felt in the last few weeks. Also, as his health improved, so did his mobility on the wheelchair. He soon got accustomed to using the remote control, and his trauma subsided! It was amazing how the electric wheelchair could provide mobility aid to somebody who could hardly turn his body from left to right even while sitting upright on it.The patient handling features were nothing less than excellent. A perfect grip enabled swift movement almost everywhere. The battery had a long life and we could take care of that for him, the rest of the wheelchair control and operation was up to him.When he could finally walk around a bit, we hired from the same place walking aids in the form of walkers. He used them for another month after he progressed from the ‘electric wheelchair’ phase. These walking aids were very well designed to give strong stability. It hardly took him any time to get accustomed to them, and he was walking around with great ease and comfort. Not a single slip, not a single jerk in the entire three weeks of use! He was using the walkers for the first time in his life, but it felt like he had been on them since his childhood!The best thing was that in spite of his very high medical and hospital bills, the hiring charges didn’t hurt his pocket.
